The file number can be found in the upper right-hand corner of the “Last Chance Animal Rescue Cat Adoption Contract.”
Last Chance Animal Rescue requires that you have your new pet checked by a vet within 72 hours of adoption to get set up on a shots schedule, monitor their general health and resolve any unforeseen issues. Have a fecal analysis performed at the first vet visit.
